An Study on The Use of Long-term Home Care Service in Agricultural Districts

Abstract
This study explores the use of long-term home care service in agricultural districts where the service providers are relatively very scarce. For this, 228 service users in 22 agricultural districts have been surveyed by telephone. The results are followed: (1) The use of the home care service was very concentrated only in assistance and bathing services. There were a few users in day and short-term custody services and nobody used night custody service; (2) Most of service users could not consume what they needed or wanted because there was no service provider. In particularity, needs for nursing service and welfare equipments was very high but the actual service uses were very low; (3) The main reason why they could not use necessary services was the scarcity of service provision; and (4) Compared with the service use at national level, agricultural districts consumed more nursing service but less day, night, and short-term custody services. Since old people in agricultural districts is relatively less healthy and limited to access to health and medical facilities, they are likely to more use nursing service. It is not certain that these results are from the scarcity of service provision in agricultural districts, but it is positive that at least those reflect on the limited service provision. With the introduction of market mechanism in the provision of public service, the number of the home care service providers has been skyrocketed since the long-term care insurance has been introduced. Agricultural districts, however, have still experienced a very limited service both in quantity and quality. It violates the service equality which is ¡®the same service for the same needs.¡¯ This study suggests several alternatives to resolve problems related to service infrastructure in agricultural districts, including the provision of service by central or local governments, the use of the existing public facilities such as local public health center, and the introduction of care management.
